Donor Recommendation Form

Making a grant from your donor advised fund is very easy.  Simply fill out the donor recommendation form below and either mail or fax it to our office.  Once we receive your form, our office will mail the grant along with a letter to your charity of choice.  Grants are typically made within 1 week.  If you need it processed sooner than that, please call our office, 785-534-1126.
